Here at St John’s CE Primary School, we are committed to creating an inclusive, supportive and nurturing environment where every child, including those with Special Educational Needs and Disabilities (SEND), is valued and enabled to achieve their full potential. We believe all children can succeed and deserve access to a broad, balanced and ambitious curriculum. We work to remove barriers to learning, promote independence, build confidence and support pupils’ academic, social and emotional development. We work in close partnership with parents, carers and external professionals to ensure every child receives the right support at the right time.

Our Intent

Here at St John’s CE Primary School, we are committed to creating an inclusive, supportive and nurturing environment where every child, including those with Special Educational Needs and Disabilities (SEND), is valued and enabled to achieve their full potential. We believe all children can succeed and deserve access to a broad, balanced and ambitious curriculum. We work to remove barriers to learning, promote independence, build confidence and support pupils’ academic, social and emotional development. We work in close partnership with parents, carers and external professionals to ensure every child receives the right support at the right time.

Our Implementation

Our SEND provision is rooted in the belief that all pupils, regardless of their individual needs, are entitled to achieve success and make progress in all areas of life. We support pupils with SEND through:

  • High-quality, adaptive classroom teaching that meets the needs of all learners.

  • Early identification of needs through observation, assessment and communication with families.

  • A graduated approach following Assess – Plan – Do – Review.

  • Personalised support plans with clear outcomes and regular reviews.

  • Targeted interventions matched to individual needs.

  • Skilled and trained support staff working closely with class teachers.

  • Reasonable adjustments and accessible resources to ensure inclusion.

  • Strong links with external agencies such as speech and language therapists, educational psychologists and health professionals when required.

  • Regular staff training to strengthen inclusive practice.

  • A positive focus on the six principles of nurture to support our children’s wellbeing, behaviour and emotional regulation.

Our Graduated Approach
(Assess – Plan – Do – Review)

We use a graduated approach to ensure that support for pupils with SEND is carefully planned, monitored and adapted to meet individual needs.

Assess

Teachers identify pupils’ needs through high-quality teaching, observation and assessment. We consider academic progress, emotional wellbeing, behaviour and communication. We work closely with parents/carers, the pupil and the SENDCo, and may seek advice from external professionals when needed.

Plan

Outcomes and targets are agreed and recorded on a personalised learning support plan. Appropriate strategies, interventions and reasonable adjustments are identified, along with clear success criteria and review dates.

Do

Support is delivered by the class teacher with the support of trained staff where appropriate. Strategies and interventions are embedded into daily classroom practice and monitored regularly.

Review

Progress is reviewed against the agreed outcomes. We gather feedback from pupils and parents and use assessment information to decide next steps. Support may be continued, adapted, or increased depending on need.

This cycle is ongoing and ensures that provision remains responsive and effective.

Our Impact

As a result of our provision:

  • Pupils with SEND make progress from their starting points.

  • Children develop confidence, independence and resilience.

  • Pupils are able to access the full curriculum and wider school life.

  • Barriers to learning are reduced through timely support.

  • Positive relationships are built between pupils, families and staff.

  • Pupils feel safe, supported and valued.

  • Successful transitions are supported between year groups and phases.

  • High aspirations are maintained for every child.

Bolton LA provides a local offer of services for children with Special Educational Needs.
